Due to administrative reasons, the current "DM Care Sugar Information" will be migrated to "HA Go- DM Care".

The HA Go-DM Care application contains knowledge about diabetes care and provides patients with the information they need for daily diabetes self-management. Users can enter their own blood sugar, blood pressure readings, and personal food diaries into the application.

After downloading HA Go, please contact your healthcare provider to push the "DM Care Sugar Information" application to you.

The HA Go – DM Care App encompasses useful tips on diabetes care providing patients with the support needed in their daily self-management of their diabetes. Users can record their self-measured blood glucose and blood pressure readings and enter records in a personal food diary in the app.
Please visit your app store to download HA Go which is available for both iOS and Android devices. Once you have downloaded HA Go, you may approach your HA health care provider to prescribe the DM Care App to you.
